Inc.Privacy and Security Policy

  1. Information Collected
    In order to access and utilize the web-based CDS Connect portal application (“Application”) hosted on Cornerstone Dental Solutions, Inc. (“CDS”) websites (collectively, the “Web Sites”), enrolled users and their representatives (“together referred to as “Subscribers”) may provide and deposit certain third party personal information and identifying data (“Subscriber Data”) at the Web Sites. This information may include but is not limited to personal health information, patient data and other personal identifiers such as legal name, address, shipping address, phone number, e-mail address, “screen name”, passwords used to access the Applications that may be covered by state and federal privacy regulation, including but limited to the Health Insurance Portability and Accountability Act of 1996 (“HIPPA”), as amended from time to time.
  2. Use of Cookies
    CDS may place Internet “cookies” on Subscriber’s computer’s hard drive to assist with the performance of the Applications. A cookie is a text file that is placed on the hard disk by a Web page server. Cookies cannot be used to run programs or deliver viruses to your computer. One of the primary purposes of cookies for the Web Sites is for session information, including information needed to perform the Applications via the Web Site functionality. Subscribers have the ability to accept or decline cookies. Most web browsers automatically accept cookies, but Subscribers can usually modify their browser setting to decline cookies if they prefer. If Subscriber chooses to decline cookies, he or she may not be able to access some of the features and/or functionality of the Web Sites and Applications.
  3. Confidentiality and Security Measures
    CDS utilizes security measures to protect against the loss, misuse, and alteration of Subscriber Data gathered from the Websites and related CDS Applications. CDS has licensed the industry-standard Security Sockets Layer (SSL) technology that permits encryption of Subscriber Data. Additionally, CDS has and will maintain appropriate security measures in place at its physical facilities to protect against the loss, misuse or alteration of Subscriber Data. CDS stores Subscriber Data in a secure environment at one or more of CDS’s authorized service providers. CDS personnel with access to this information have been advised of the importance of maintaining the confidentially of such information and of using it only for the purposes described in this Privacy and Security Policy and/or Subscriber’s Subscription and Use Agreement with CDS.CDS will not share, sell, or rent information that it collects with any third party without Subscriber’s prior permission, or unless CDS believes, in good faith, that such release is reasonably necessary to (i) comply with applicable law and privacy regulations, (ii) enforce the terms of the Subscription and Use Agreement, or (iii) protect the rights, property or safety of CDS or others.The Websites and related Applications may provide links to third-party sites for Subscriber’s convenience and information. If third party sites are accessed via such links, Subscriber will leave the Websites and related applications. CDS does not control third-party sites or third party privacy practices which may differ from this Privacy and Security Policy. CDS does not endorse or make any representations about third-party sites. Any personal information Subscriber chooses to give to such third parties is not covered by CDS’s Privacy and Security Policy.
  4. No Liability for Acts of Third Parties
    Although CDS will use all reasonable efforts to safeguard the confidentiality and security of Subscriber Data, transmissions made by means of the Internet cannot be made absolutely secure. CDS will have no liability for disclosure of Subscriber Data due to errors in transmission or unauthorized acts of third parties beyond its reasonable control.
  5. UpdatesCDS reserves the right to change or update this Privacy and Security Policy at any time. Any changes or updates will be effective immediately upon posting to the Web Sites, and Subscriber’s use of any of the Web Sites following such posting constitutes Subscriber’s acceptance of the Privacy and Security Policy as so changed or updated.
